Referrals Instructions for Completion Criteria for Acceptance of Referrals GENERAL POINTS Our Advocates are NOT ADVICE WORKERS, nor are they PROFESSIONAL EXPERTS IN SPECIALISED FIELDS. Although advocates may sometimes have appropriate professional expertise their role is to act as an unpaid private citizen - a ‘friend’ or ‘facilitator’ to ACCESS RESOURCES, rather than be a specific provider. The basic premise is to independently advocate on behalf of their partner on a one-to-one/group basis as if their partner’s interests were his or her own. Who can fill in this form? If possible this form should be filled in by the person needing an advocate. Where this is not possible, this form can be completed by whoever knows the person best, eg. friend, relative, key worker. l referrals must initially be sent to Liz Naylor at our Chesterfield office.
